C5a is a polypeptide consisting of 74 amino acids (11KD in size) derived from the N-terminus of the alpha chain. The life of C5a is very transient and rapidly cleaves in serum into a more stable but still biologically active C5a-desArg (also known as acylation stimulating protein, ASP). Therefore, determining the C5a-desArg content to indicate that the sample complement activation level is reliable.

C5a as a powerful anaphylatoxin can cause smooth muscle contraction, vasodilation, increased vascular permeability, degranulation of basophils and mast cells, and lysosomal enzyme release. In addition, C5a is a potential chemokine for eosinophils, basophils, and monocytes.

Inflammatory responses involving C5a include: sepsis caused by Gram-negative bacteria, trauma, ischemic heart disease, post-dialysis syndrome, and various autoimmune diseases. An increase in C5a may indicate an increased risk of cardiovascular disease in patients with atherosclerosis. In addition, C5a is closely associated with capillary leak syndrome in leukemia children after bone marrow transplantation. The C5a content in urine can predict the occurrence of acute rejection after renal transplantation.
